Recovery, Resiliency and Reintegration (All age groups)

RRR Program Overview:

The RRR services are provided to all age groups across the lifespan to reduce significantly impairment that includes three major components; Clinic-based, Community-based, and Well-being services geared towards complete integration into the community.


Program Details: 

  • Clinic-based services primary goal is to provide services in an Outpatient setting for individuals directly at the center for intensive mental health services as apart of recovery and resiliency tenets. 
  • Community-based services are provided to individuals who either cannot utilize clinic-based and/or advanced in their treatment and beginning to move towards more positive interactive integration into the community.
  • Well-being services are provided primarily to support mental health and re-engagement in meaningful community activities to maximize total well-being integration.

RRR Services for Children:
  
  • All Child RRR services will include the array of specialty mental health services:
  • Linkage and case management support, including linkage for co-occurring medical, dental, vision or other health care needs o Consultation with age specific specialty treatment providers
  • Intensive Care Coordination (ICC) o Intensive Home Based Services (IHBS)
  • Specialized assessment and treatment interventions for clients with co-occurring disorders (i.e. mental illness, substance abuse, physical health, including mental health service co-location in health settings, substance use facilities, schools, recreation facilities, libraries, etc.).
  • Parent and Peer support, and educational classes based on the surveyed needs of the community.
  • Self-help support groups to meet the needs of the community, especially parents/caregivers of children who have an open case.
  • Co-occurring disorder educational, preventative and recovery support groups for children/youth and caregivers that address, signs of addiction, impact of addiction, coping strategies, tools of recovery, motivation to change and relapse prevention. Provide guidance and support towards behavioral changes and recovery.

RRR Services for Transitional Age Youth (TAY):   
  • All TAY RRR services will include Peer Support/Peer Led Support Groups and Supported employment.
  • Community-Based services include Primary Care Collaborations, Juvenile Justice Diversion, and Probation Camps.
  • Clinic-Based services include Cognitive Behavioral Therapy, Dialectical Behavior Therapy, and Trauma Services.
  • Well-Being & Peer Run Services include TAY Drop-in Centers and services to increase well-being & reintegration.

RRR Services for Adults:   
  • All Adult RRR services will include Supported Employment and Peer Support.
  • Community-Based services include Primary Care Collaborations, at home and in the community.
  • Clinic-Based services include Cognitive Behavioral Therapy, Dialectical Behavior Therapy, and Trauma Services.
  • Well-Being & Peer Run Services include Peer Support Services and services to increase well-being & reintegration.

RRR Services for Older Adults:   
  • Community-Based services include Cognitive Screening, Medication Evaluation/Management Psychotherapy, Case Management, Peer Support Outreach & Engagement, and Crisis Intervention.
  • Clinic-Based services include Cognitive Screening, Linkage/Collaboration with Primary and Specialty Healthcare, Use of Evidence Based Practices: Seeking Safety, Interpersonal Psychotherapy for Depression, Cognitive Behavioral Therapy & Problem Solving Therapy.
  • Well-Being & Peer Run Services include Cognitive Screening, Peer Support Services, Chronic Disease Self-Management and other Community-Based Services, and services to increase well-being & reintegration.
