Full Service Partnership Mental Health Clinician

We are currently seeking an experienced Field Based Full Service Partnership Mental Health Clinician . The Clinician is responsible for providing community based mental health services to TAY and Adults in our Full Service Partnership (FSP) program. The target focal population of FSP participants consist of individuals with severe and persistent mental illness who have history of and/or are at high risk for homelessness, incarceration and/or institutionalization.

Full Service Partnership Case Manager

Under the direction of the Clinical Supervisor, provides comprehensive and intensive case management services to adults with severe and persistent mental illness. Case management services include linkage to community resources such as housing, medical, educational and vocational services. In addition, provides rehabilitation services that focus on improving daily functioning through an integrated recovery approach that improves daily living skills and supports consumers in meeting their goals.

Spanish Speaking Mental Health Therapist- Children and TAY

Licensed Psychiatric Technician/LVN

We are currently seeking an experienced California Licensed Psychiatric Technician (LPT) or Licensed Vocational Nurse (LVN) for the Adult FSP and Outpatient program. The FSP program is funded by Proposition 63, the Mental Health Services Act. This FSP Program is for adults ages 26-59 who would benefit from an intensive service program for persons diagnosed with a mental illness who would like to work in partnership with a service provider team to develop and achieve individualized recovery-focused goals. Adults in this program have been challenged by homelessness, as well as frequent hospitalizations and/or incarcerations. The best candidate for this position will make it their number one priority to ensure our members are receiving the highest quality of care that can be provided and ensure that all staff are delivering that same service.

Spanish Speaking Child and Family Specialist

The Spanish Speaking Child & Family Specialist is responsible for providing direct one-on-one behavioral interventions to assist a child/youth in eliminating destructive activities, minimizing impulsivity, and to promote pro-social functioning as identified in their individualized services plan. The Child and Family Specialist provides a positive role model and uses a flexible strengths based approach to assist the child/youth/family in development of necessary daily living skills and competencies that are required to achieve permanency and successfully live in the community. This individual works directly with the children and families in the home, community, or wherever is most convenient for the family. Assists with helping child/youth work towards achieving a positive relationship between family members and maintaining a permanent and successful placement.
